Earliest reference I know of is H.L. Gold’s 1953 novelette “No Charge For Alterations.” Doctors have machines which can change the patient’s body and mind in many ways, instantly. However: the girl (ten years old, if I recall correctly) who wants to become a boy is turned down because the colony world needs to maintain its sex ratio.
